We are seeking a skilled Home Health Registered Nurse (RN) Case Manager to join our team at Interim HealthCare. As a Home Health RN, you will play a vital role in providing high-quality patient care and advocating for the needs of our patients.
Key Responsibilities- Provide home-based care to patients with illnesses, injuries, and chronic diseases, often after discharge from a hospital, rehab, or nursing facility.
- Work collaboratively with a team of physicians and oversee LPNs, CNAs, and aides providing patient care.
- Conduct thorough patient assessments, document progress, and report changes to the patient's physician.
- Develop and implement personalized care plans, educating patients on their treatment, medication, therapy, diet, and exercise.
- Coach family members on the patient's care plan, medication, and home safety.
- Ensure patient goals are met and coordinate discharge from services.
- Manage a caseload of 30-35 patients, with assigned visits per day based on a point system, averaging 5-7 visits per day.
- Graduate of an accredited nursing program and active RN license in Ohio.
- Minimum of 1 year of nursing experience.
- CPR certification.
- Clinical proficiency in nursing specialty, strong communication skills, and compassion.
- Home Health experience preferred but not required.
