POSITION SUMMARY:

The Healthcare Clinic Nurse plays a vital role in delivering medical care and related services within the Health Care Center of Compass Community Health, adhering to the regulations set forth by the state of Ohio Board of Nursing and the policies established by Compass Community Health.

KEY RESPONSIBILITIES:

Key responsibilities encompass the essential duties necessary for the role, which must be performed effectively with or without reasonable accommodations.

  1. Administers patient care and nursing services in accordance with the guidelines established by the state of Ohio Board of Nursing.
  2. Collaborates with the Medical Clinic Office Manager to oversee inventory management within the health center.
  3. Works alongside other health center personnel to ensure a clean, safe, and organized environment for both staff and patients.
  4. Assists the Medical Clinic Office Manager in maintaining up-to-date policies, procedures, protocols, and compliance standards required by various regulatory bodies.
  5. Represents the department and agency positively through professional interactions, appearance, and demeanor.
  6. Upholds patient confidentiality in compliance with regulations, safeguarding the privacy of healthcare consumers in all communications and documentation.
  7. Utilizes electronic health records to access pertinent information and document care accurately.
  8. Considers clinical and cost-effectiveness in decision-making regarding service organization and delivery.

ADDITIONAL FUNCTIONS:

Additional functions are tasks that, while not exclusive to this position, contribute to the overall efficiency of Compass Community Health.

  1. Provides assistance with administrative tasks as necessary.
  2. Performs other duties as assigned by the Medical Clinic Office Manager.
  3. Establishes and maintains effective working relationships with a diverse range of individuals, including patients, family members, and other healthcare providers.
  4. Communicates the roles and responsibilities of each team member to patients and families, explaining how they collaborate to deliver services.
  5. Maintains fluency in healthcare terminology.
  6. Respects and responds to leadership demonstrated by healthcare providers within a team setting.
  7. Facilitates collaborative care by sharing relevant information with authorized individuals while adhering to HIPAA regulations.
  8. Assesses the patient's family and social support systems that may influence healthcare outcomes.
  9. Ensures effective communication among patients, family members, and healthcare providers.
  10. Applies wellness and recovery models in understanding consumer health and healthcare.
  11. Identifies signs of abuse, neglect, domestic violence, and other forms of trauma in patients.
  12. Recognizes common health conditions and crises, understanding their symptoms and treatments.
  13. Ensures the flow of information among healthcare consumers, family members, and providers.
  14. Supports patients in exploring and accessing complementary and alternative health services.
  15. Provides education, guidance, and support to family members and caregivers.
  16. Coordinates with health plans to identify and address patient and population health needs.
  17. Encourages adherence to healthcare plans among patients and families.
  18. Offers support in managing the consequences of illness or injury.
  19. Values and promotes diversity within the team composition.
  20. Plans and delivers services with an understanding of the healthcare needs of the served population.
  21. Sets and pursues individual and team-based improvement objectives.
  22. Conducts screenings using SBIRT and PRAPARE tools to assess substance use and social determinants of health.
  23. Coordinates referrals to behavioral and mental health providers as needed.
  24. Participates in team meetings and quality improvement initiatives related to Patient Centered Medical Home practices.
  25. Engages in data collection and outcome reporting as part of quality programs.

QUALIFICATIONS:

  1. Current licensure as a Licensed Practical Nurse in the state of Ohio.
  2. Experience in a patient/family healthcare environment is preferred.
  3. Knowledge of addiction-related issues is advantageous.
  4. Maintains necessary agency training requirements.
