Key Responsibilities
  • Assess and coordinate patient discharge planning needs with healthcare team members.
  • Perform point-of-care testing and provide skilled nursing care within established plans of care.
  • Collaborate with physicians and other healthcare professionals to develop and implement comprehensive care plans.
  • Document visit notes, treatments, and care in a timely and accurate manner.
  • Respond to patient and family inquiries, addressing needs, issues, or concerns.
Requirements
  • Graduate of an accredited school of nursing or equivalent education.
  • Current RN licensure in the state of employment.
  • Basic Life Support (BLS) certification.
  • Valid driver's license and reliable transportation.
