We are seeking a skilled and compassionate Registered Nurse to join our team as a Home Health and Hospice Nurse Coordinator. In this role, you will be responsible for coordinating and facilitating interdisciplinary plans of care for patients in their homes, community-based residential facilities, and other settings.
Key Responsibilities- Assess and coordinate patient discharge planning needs with healthcare team members.
- Perform point-of-care testing and provide skilled nursing care within established plans of care.
- Collaborate with physicians and other healthcare professionals to develop and implement comprehensive care plans.
- Document visit notes, treatments, and care in a timely and accurate manner.
- Respond to patient and family inquiries, addressing needs, issues, or concerns.
- Graduate of an accredited school of nursing or equivalent education.
- Current RN licensure in the state of employment.
- Basic Life Support (BLS) certification.
- Valid driver's license and reliable transportation.
