Job Title: Supportive Housing Counselor

At Mental Health Association of Nassau County, we are seeking a dedicated and compassionate Supportive Housing Counselor to join our team. As a Supportive Housing Counselor, you will play a vital role in providing supportive services to individuals with mental illness, helping them achieve their personal recovery goals and live independently.

Key Responsibilities:
  • Provide weekly visits to participants in the Supportive Housing Program for the first 4 weeks of admission.
  • Conduct face-to-face contact with participants at least once a month, with additional telehealth contact as needed.
  • Offer supportive services, including daily living skills, assessment, and linkage to community services, as well as teaching participants how to be responsible tenants in an apartment setting.
  • Assist residents with gathering and completing financial documentation for recertifications, leases, and other purposes.
  • Provide crisis management services and coordinate care for participants who are hospitalized.
  • Help participants develop realistic and measurable personal recovery goals that are culturally sensitive to their beliefs and background.
  • Assist with admissions and discharges, including packing and unpacking belongings and processing paperwork.
  • Act as a liaison between participants, landlords, and treatment team members.
  • Conduct fire safety and disaster preparedness drills in a safe and professional manner.
  • Ensure the health and safety of participants' living areas.
  • Conduct medication reviews quarterly or as needed.
  • Complete reports using Electronic Medical Records and ensure timely completion of required documentation.
  • Participate in staff meetings, including one-on-one and group supervision.
  • Attend required meetings, seminars, and in-service trainings.
  • Adhere to the agency's Quality Improvement and Compliance policies and procedures.
Requirements:
  • Required: Bachelor's Degree in a Human Service field.
  • CPR and First Aid certification.
  • Minimum of 2 years of experience working with individuals diagnosed with mental illness.
