We are seeking a highly skilled Case Manager to join our team at Johns Hopkins Health System. As a Case Manager, you will play a critical role in coordinating patient-centered care across the continuum, ensuring seamless transitions and progression in care.
Key Responsibilities- Collaborate with patients, families, social workers, physicians, and the interdisciplinary team to provide leadership and advocacy in care coordination.
- Manage clinical resources and transition planning for patients within an assigned caseload from pre-admission through post-discharge.
- Identify and eliminate barriers to the delivery of clinical services, working closely with the patient, family, and care team.
- Develop and implement effective care plans, ensuring optimal patient outcomes and satisfaction.
- Communicate effectively with patients, families, and healthcare providers to ensure timely and efficient care transitions.
- Completion of an accredited RN Nursing Program; BSN preferred.
- Minimum 3-5 years of nursing experience required, with a focus on psychiatric care.
- Preferred experience in case management, discharge planning, or utilization review.
- Accredited Case Manager-Registered Nurse (ACM-RN)/Certified Case Manager (CCM) preferred.
- Valid Maryland Nursing License required.
