Empyrean Hospice Job Description
We are seeking a compassionate and dedicated Hospice Care Specialist to join our team. As a member of our interdisciplinary group, you will provide high-quality clinical care and support to patients and their families.
Key Responsibilities:
- Represent Empyrean Hospice with professionalism and compassion
- Serve as a member of the patient's clinical care team
- Promote quality, continuity, and safety in patient care
- Perform personal care needs, including bathing, oral hygiene, and feeding assistance
- Observe patient conditions and report observations and care performed
- Provide positive communication and confidentiality
- Adhere to documentation and care procedures
Requirements:
- Self-directing with ability to work with little supervision
- Understands hospice philosophy and is secure with issues of death and dying
- Able to lift, position, and transfer patients and supplies
- Meets state training requirements
- Completed 75 hours of classroom and practical training
