The ideal candidate will possess a Graduate degree from an accredited Diploma, Associate or Baccalaureate School of Nursing, with a minimum of two years of experience as an APN in a clinical care setting, preferably in hospice or home health. The successful candidate will demonstrate comprehensive knowledge of pharmacology, palliative care, and end-of-life care, as well as excellent interpersonal skills.

Key Responsibilities:

  • Collaborate with the Interdisciplinary Team to ensure quality care and palliative treatment of terminal disease.
  • Consult with and educate patients and families regarding disease process, self-care techniques, and end-of-life care.
  • Perform and document visits to patients and families based on the plan of care and patient/family need.
  • Interact with attending and consulting physicians as necessary regarding palliative care, in collaboration with the hospice physician.
