DePaul Community Resources is seeking a dedicated Case Management Assistant for a full-time position.


POSITION SUMMARY:

The Case Management Assistant plays a crucial role in managing, updating, and finalizing documentation, including case notes and various records within the ID/DD Services Division. This role is designed to assist specific case management documentation tasks for the Residential Coordinators in designated office locations. The position is primarily remote, with occasional office visits for meetings as necessary.


SUPERVISION:

This position reports directly to the Regional Manager.


KEY RESPONSIBILITIES:

The following responsibilities are essential functions of the role:

  • Review and analyze documentation such as monthly progress notes and assessments, providing constructive feedback to the assigned Residential Coordinator and Supervisor.
  • Conduct thorough reviews of assigned documentation to facilitate quarterly person-centered reviews for submission to the designated Residential Coordinator and Supervisor.
  • Draft initial versions of annual plans by compiling information from assessments and evaluations, forwarding drafts to the assigned Residential Coordinator and Supervisor for completion.
  • Ensure compliance with deadlines set by the electronic health record system and supervisory guidelines.
  • Complete necessary training as assigned.
  • Provide insights on trends or areas of concern identified in specific case records or documentation.

QUALIFICATIONS:

The ideal candidate will possess:

  • An Associate's degree in a human services field and one year of experience with the ID/DD population, or two years of equivalent experience.
  • Strong knowledge of records administration techniques and the structure of the English language.
  • Proficiency in Microsoft Office applications and virtual meeting platforms.

WORK ENVIRONMENT:

This role is primarily office-based with occasional fieldwork. The work schedule is generally during weekdays, with variations in daily hours.


PHYSICAL DEMANDS:

Physical activities required include the ability to operate office equipment, effective communication, and mobility for various tasks.


ADDITIONAL REQUIREMENTS:

Successful candidates may be required to undergo background checks and screenings as part of the hiring process.
