We are currently seeking a Case Management Specialist to enhance our expanding team. The Case Management Specialist will primarily focus on the following responsibilities:

  • Ensure confidentiality while adhering to company, state, federal, and HIPAA regulations.
  • Document interactions for insurance purposes.
  • Maintain a professional and positive demeanor towards clients, colleagues, and volunteers.
  • Create a safe and supportive environment for clients.
  • Demonstrate a genuine commitment to client recovery.
  • Provide transportation for clients to necessary appointments.
  • Monitor and support clients' emotional health and behavior.
  • Facilitate connections between clients and family or community resources.
  • Assist clients in liaising with legal entities when required.
  • Encourage clients to achieve their personal goals.
  • Conduct weekly meetings with clients.
  • Support residents in developing long-term care plans and accessing educational resources.

Qualifications and Experience Required:
  • Bachelor's degree in social work or a related human services field is mandatory.
  • One year of post-graduate experience or a master's degree is essential.
  • A valid driver's license is required.
  • Basic proficiency in computer skills is necessary.
  • Ability to compile and submit reports effectively.
  • Self-motivated with strong documentation capabilities.
  • Excellent written and verbal communication skills are crucial.
  • Proficient in using computers and search engines.
  • Ability to manage crisis situations calmly and supportively.
  • Maintain a positive and professional attitude towards clients and colleagues.
