Gentiva Hospice is a leading provider of hospice care, and we are seeking a compassionate and skilled Hospice Registered Nurse to join our team.

About the Role:

As a Hospice Registered Nurse, you will provide high-quality patient care and support to individuals and their families in their homes or in our care centers. You will work closely with our interdisciplinary team to develop and implement individualized care plans that address the physical, emotional, and spiritual needs of our patients.

Responsibilities:

  • Provide direct patient care services, including administering medications, wound care, and other treatments as needed.
  • Conduct thorough assessments of patients' physical, emotional, and spiritual needs, and develop and implement individualized care plans.
  • Collaborate with our interdisciplinary team to ensure seamless care coordination and communication.
  • Provide education and support to patients and their families on disease management, symptom control, and end-of-life care.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.

Requirements:

  • RN license in the state of employment (or eligibility to obtain).
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology.
  • Current driver's license and automobile insurance.
  • Current CPR certification.
