Pediatric Specialty Infusion Care Specialist

Soleo Health Inc is seeking a skilled Per Diem Pediatric Specialty Infusion Care Specialist to provide expert care to patients in the McAllen, TX area.
Estimated Salary Range: $60,000 - $90,000 per yearThis role involves providing primary patient care for assigned patients, including assessments, care planning, evaluation, and education of patients receiving infusion therapy and other nursing services in a home or alternate site setting.
Responsibilities:- Directly manage care for assigned patients, including initiation and maintenance of all communications with care team members
- Evaluate and educate patients/care partners on safely administering home infusion therapy
- Evaluate the home environment and identify necessary interventions
- Develop and evaluate patient Care Maps and respond to therapy changes
- Discharge primary patients, complete discharge summaries, and maintain caseload scheduling and communications
- Bilingual skills are mandatory
- Pediatric experience is required
- Bleeding disorder experience is preferred
- RN Degree with BSN Preferred
- Advanced certification in specialty areas of practice is preferred but not required
- On-call participation, nights, weekends included, and emergency availability
