Home Infusion Nurse Specialist

We are seeking an experienced Home Infusion Nurse to join our team at Soleo Health Inc. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and ensuring the safe administration of infusion therapy in patients' homes.
This is a per diem opportunity with variable hours, approximately 5 hours a week, allowing for flexibility in your schedule. You will work independently to manage your caseload, communicate with our clinical team, and document patient visits.
Responsibilities- Provide direct patient care, including assessments, care planning, evaluation, and education on infusion therapy and nursing services.
- Evaluate and educate patients and their caregivers on the safe administration of home infusion therapy.
- Conduct thorough assessments of the home environment to identify potential interventions and ensure a safe treatment area.
- Develop and maintain individualized Care Maps with our clinical team to optimize patient outcomes.
- Coordinate discharge summaries and complete necessary documentation.
- Bilingual proficiency (English/Spanish) is required, as well as pediatric experience.
- A minimum of 3 years of nursing experience, preferably in infusion therapy or ICU settings.
- RN degree; BSN preferred.
- Advanced certifications in specialty areas, such as CRNI or ONS, are desirable but not mandatory.
- Valid state-issued driver's license with no significant restrictions.
- Competitive hourly rate: $65-$80 per hour (estimated annual salary: $134,400 - $166,400)
