Nurse Care Manager

Responsibilities- Manage all aspects of the nursing process, including assessment, planning, implementation, and evaluation
- Develop and implement individualized care plans that address the unique needs of each patient
- Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes
- Maintain accurate and detailed records of patient interactions, treatments, and progress
- Participate in ongoing education and training to stay current with best practices and advancements in the field
To be successful in this role, you will need:
- A valid/Current license to practice as a RN in the Commonwealth of Massachusetts
- BLS certification
- Graduate of an accredited school of Nursing
- Preferred 2+ years Med/Surg experience
- Bachelor's Degree preferred
- ACLS preferred
We offer a competitive salary range of $85,000 - $110,000 per year, based on experience, plus a comprehensive benefits package that includes medical, dental, and vision coverage, life insurance, employee assistance programs, and opportunities for professional growth and development.
