Position Overview

This full-time position for a Hospice Nurse Practitioner at Hospice East Bay involves weekend rotations. The Nurse Practitioner will serve as the primary clinician for our Palliative Care program, conducting home visits, coordinating necessary care, and guiding other members of the interdisciplinary team. The role includes creating and updating care plans tailored to the goals of the patients and their families.

Key Responsibilities:

  • Participate in palliative care consultations as required.
  • Provide consultation to patients, families, and staff regarding end-of-life care and palliative services.
  • Support the education and training of staff and engage in community outreach.
  • Maintain confidentiality regarding all patient and organizational information.
  • Assist in the coordination and execution of Physician Orders for Life-Sustaining Treatment (POLST) and Advance Directives.
  • Coordinate patient care in alignment with the established goals of care.
  • Attend weekly interdisciplinary team meetings.
  • Contribute to the development of comprehensive care plans as a member of the interdisciplinary team.
  • Consult with the Chief Medical Director as necessary to ensure adherence to best practices.
  • Document all patient interactions in the Electronic Medical Record (EMR) according to established standards.
  • Conduct home visits when a physician or NP-level visit is warranted.
  • Mentor new Nurse Practitioners in the standards of the Palliative Care Program.
  • Engage with hospice physicians or the Quality Assessment Performance Improvement (QAPI) team on quality improvement initiatives.
  • Adhere to confidentiality policies regarding all patient and organizational matters.
  • Perform additional duties as assigned.

Qualifications:

  • Valid California driver's license.
  • Current auto insurance.
  • Exceptional communication and interpersonal skills.
  • In-depth knowledge of home care, hospice, palliative care, advance directives, pain management, symptom control, and discharge planning.
  • Current CPR certification.
  • Current BLS/ACLS certification.

Experience and Education:

  • Valid California Nurse Practitioner license.
  • Relevant Master's degree.
  • Active DEA Certificate for controlled substances (schedule II and higher).
  • Minimum of two years of experience in palliative care with end-of-life patients.
  • Advanced Practice Certified Hospice and Palliative Nurse (ACHPN) preferred.
  • At least two years of experience in Hospice and/or Home Care preferred.
