Position Overview
This full-time position for a Hospice Nurse Practitioner at Hospice East Bay involves a weekend rotation. The Nurse Practitioner will serve as the primary clinician for our Palliative Care program. Responsibilities include conducting home visits, coordinating necessary care, leading the interdisciplinary team, and developing and updating care plans to align with the goals of patients and their families.
Key Responsibilities:
- Participate in palliative care consultations as needed.
- Provide expert consultation to patients, families, and staff regarding end-of-life matters and palliative care.
- Support the education and training of staff and engage in community outreach.
- Maintain confidentiality regarding all patient and organizational information.
- Assist in the coordination and completion of Physician Orders for Life-Sustaining Treatment (POLST) and Advance Directives.
- Coordinate patient care in accordance with established goals.
- Attend weekly interdisciplinary team meetings.
- Contribute to the development of comprehensive care plans for patients.
- Consult with the Chief Medical Director as necessary to ensure adherence to best practices.
- Document all patient interactions in the Electronic Medical Record (EMR) according to established standards.
- Conduct home visits when necessary or upon request.
- Mentor new Nurse Practitioners in Palliative Care standards and best practices.
- Engage with hospice physicians or the Quality Assessment Performance Improvement (QAPI) team on quality improvement initiatives.
- Uphold confidentiality policies regarding all patient and organizational information.
- Perform additional duties as assigned.
Qualifications:
- Valid California driver's license.
- Current auto insurance.
- Strong communication and interpersonal skills.
- Knowledge of home care, hospice, palliative care, advance directives, pain management, symptom control, and discharge planning.
- Current CPR certification.
- Current BLS/ACLS certification.
Education and Experience:
- Valid California Nurse Practitioner license.
- Relevant Master's degree.
- Active DEA Certificate for controlled substances (schedule II and higher).
- Minimum of two years of experience in palliative care with end-of-life patients.
- Advanced Practice Certified Hospice and Palliative Nurse (ACHPN) certification preferred.
- At least two years of experience in Hospice and/or Home Care preferred.
