We offer compassionate end-of-life services to patients facing terminal illnesses, while also providing emotional, spiritual, logistical, and bereavement support to their families and caregivers.
Position Overview:
The Registered Nurse (RN) Case Manager plays a crucial role in our interdisciplinary team (IDT). This position is responsible for assessing and addressing the physical, psychosocial, environmental, safety, and developmental needs of patients and their families. The RN Case Manager will tailor the patient's care plan based on identified needs, ensuring continuous evaluation and implementation of care in collaboration with the attending physician and the medical director.
Key Responsibilities:
- Recognize and respond to signs of active dying.
- Facilitate communication and collaboration among patient care teams, including social workers and support coordinators.
- Conduct comprehensive assessments of patients' pain and develop individualized pain management strategies.
- Anticipate, prevent, and manage undesirable symptoms.
- Address comfort care needs effectively.
- Educate and support patients, families, and caregivers through effective teaching methods.
- Document nursing assessments, care interventions, and patient outcomes accurately.
- Uphold the dignity, confidentiality, and privacy of patients during their end-of-life journey.
- Respect and support the unique spiritual and cultural beliefs of each patient.
- Deliver holistic, family-centered care across various treatment settings to enhance quality of life.
- Collaborate with the interdisciplinary team and other stakeholders involved in patient care.
- Coordinate care plans with after-hours and crisis care staff.
- Participate in interdisciplinary team meetings, presenting relevant patient information.
- Update comprehensive assessments and track patient progress regularly.
- Complete patient-related documentation promptly after each shift.
- Adhere to state and federal regulations, as well as accreditation standards.
- Provide education and training to patients and primary caregivers as needed.
- Follow evidence-based practices endorsed by the agency.
- Manage care transitions effectively.
- Oversee care coordination for transfers to higher levels of care.
Additional Case Management Duties:
- Maintain communication with the Team Manager regarding patient acuity and workload.
- Ensure effective hand-off communication within the agency.
- Assist in training new staff members as directed.
- Supervise the performance of LVNs and Home Health Aides.
- Attend relevant in-service training sessions.
- Monitor medication reconciliation processes in compliance with Medicare and accreditation standards.
Qualifications:
- Current California Registered Nurse License is required.
- A minimum of one year of experience in an acute clinical setting is necessary.
- CPR certification is mandatory.
- Possession of a valid California Driver's License and access to a reliable vehicle is essential.
- Experience in hospice and/or home care is preferred.
- Hospice and Palliative Care Nurse Certification is highly desirable.
- Knowledge and expertise in IV therapy are advantageous.

Work Schedule:
This position typically follows a Monday to Friday schedule from 8:00 AM to 4:30 PM.
