We are seeking a highly skilled and motivated At-Home Physical Therapist to join our team. The successful candidate will be responsible for providing high-quality physical therapy services to patients in their homes, working closely with patients to develop personalized treatment plans, and maintaining accurate records of patient care.
Responsibilities- Conduct Initial Evaluations: Conduct thorough initial evaluations of patients to identify their needs and goals.
- Develop Treatment Plans: Develop personalized treatment plans that address patients' unique needs and goals.
- Provide Physical Therapy Services: Provide high-quality physical therapy services to patients in their homes, including exercises, modalities, and other interventions.
- Maintain Accurate Records: Maintain accurate records of patient care, including progress notes, treatment plans, and other relevant information.
- Communicate with Patients and Families: Communicate effectively with patients and their families to ensure that they understand their treatment plans and progress.
- Master's or Doctoral Degree in Physical Therapy: A master's or doctoral degree in physical therapy is required.
- Current Licensure: Current licensure as a physical therapist in the state of practice is required.
- Experience: Experience working as a physical therapist in a home health or outpatient setting is preferred.
