Job Title: Home Health Physical Therapist

BAYADA Home Health Care is seeking a skilled Home Health Physical Therapist to join our team in York, Pennsylvania.

About the Role:

We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our clients in their homes. As a Home Health Physical Therapist, you will be responsible for assessing and treating clients with various physical and mobility issues, developing and implementing individualized care plans, and educating clients and their families on rehabilitative care and activities.

Responsibilities:
  • Conduct home visits to assess clients' physical and mobility needs
  • Develop and implement individualized care plans to improve clients' physical function and mobility
  • Provide manual therapeutic exercises, gait training, balance, and other interventions as needed
  • Educate clients and their families on rehabilitative care and activities
  • Document observations, interventions, and evaluations in a state-of-the-art touch pad tablet
Qualifications:
  • Current PA Physical Therapist license
  • Minimum of one year of recent work experience as a Physical Therapist
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
  • Ability to work independently and manage time effectively
  • Strong interpersonal, organizational, and problem-solving skills
