Metro Physical & Aquatic Therapy is a dynamic and forward-thinking company that offers a comprehensive range of therapy services. We are seeking a motivated and passionate Physical Therapist to join our team in a per diem capacity. As a Physical Therapist with Metro, you will have the opportunity to work with a diverse range of patients and develop individually designed programs to restore overall function and independence in the home setting.
**Key Responsibilities:**
* Evaluate patients' needs for physical therapy and develop appropriate treatment plans
* Provide skilled therapy to patients in their homes, with a focus on patient care and quick EMR
* Develop plans of care and goals in consultation with physicians, families, and patients
* Timely and accurate documentation of visits
* Effective communication with family members and other team members

**Requirements:**
* Home health experience is preferred but not required
* NY Physical Therapist License or eligible
* Evaluates a patient's need for physical therapy and develops an appropriate treatment plan
* Provide appropriate care and treatment to patients based on physician care plan, agency policy, and professional standards of care
* Instructs family members, PTA's, Home Health Aides, and other caregivers in appropriate physical therapy techniques
**Certifications Required:**
* CPR/First Aid
