Patient Navigator II

Fred Hutchinson Cancer Center is seeking a skilled Patient Navigator II to join our team. As a Patient Navigator II, you will play a critical role in ensuring that patients receive high-quality, patient-centered care. Your primary responsibility will be to assess and assist patients, families, and caregivers in overcoming practical, tangible barriers to care.
Key Responsibilities- Assess patients' needs and develop personalized care plans to address barriers to care.
- Collaborate with healthcare professionals and community service providers to facilitate timely care and arrange supportive services as needed.
- Provide cultural brokerage services to ensure that patients receive culturally appropriate care and services.
- Develop and maintain relationships with diverse community organizations to promote access to care at Fred Hutch.
- Participate in care coordination and provide patient education on the multi-disciplinary nature of cancer care and survivorship.
- High school diploma or equivalent required.
- Three years of experience in social services or a health-related field.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Ability to work independently and as part of a team, with a strong focus on patient-centered care.
- Technical skills in Microsoft Office and other programs, with the ability to learn new software and systems.
- BA/BS degree in a related field.
- Oral and written fluency in a language other than English.
