Job Title: Registered Nurse

The Registered Nurse is a vital member of our team, responsible for accepting and completing Home Care referrals received. This role involves collaborating with referring institutions, physicians, patients, and caregivers to develop appropriate Home Care plans. Additionally, the Registered Nurse works with established referral sources and develops new sources for Home Care referrals, increasing awareness of the scope of services that Home Care can provide for physicians and other healthcare personnel.

Responsibilities:
  • Screen all referrals for appropriateness of Home Care services.
  • Collaborate with the Home Health team to identify potential patients for Home Care services.
  • Review hospital charts and obtain necessary information.
  • Collaborate with Case Managers from area hospitals in the coordination of Home Care services.
  • Demonstrate a professional attitude and ethical conduct in compliance with departmental standards.
  • Attend a minimum of six in-service programs annually.
  • Involves patients and caregivers actively in goal setting.
  • Collaborate with staff regarding patient plan of care and progress.
  • Demonstrate proper teaching techniques in the development and implementation of individualized age-specific teaching plans and seek assistance as needed.
  • Teach and supervise ancillary staff to gain knowledge and skills to appropriately carry out assigned tasks.
  • Serve as a mentor to staff.
  • Confer with physicians and other members of the healthcare team to formulate a plan of care, including hospital case managers/discharge planners for tracking of hospitalized patients.
  • Provide information and referral services as needed to physicians and caregivers calling from the community and follow up on appropriate Home Care referrals.
  • Educate potential referral sources regarding services.
  • Explain services to referral sources, patients, and caregivers in a concise manner.
  • Seek opportunities to enhance Home Care knowledge.
  • Incorporate QI recommendations into clinical practice.
  • Assist in the evaluation and revision of departmental standards of care and/or practice.
  • Demonstrate problem-solving in the decision-making process.
  • Serve as a liaison while at facilities between hospitals and the Home Care Agency.
  • Establish good working relationships with all members of the healthcare team.
  • Attend monthly staff meetings.
  • Verify and confirm health insurance coverage for Home Care services and document coverage on the intake sheet.
  • Demonstrate competency in the interpretation of a patient's self-report or behavior; interpret information with an understanding of the cognitive, physical, emotional, and chronological maturation process.
  • Initiate the Home Care referral process in a knowledgeable, skillful, consistent manner with Home Care staff and hospital case managers.
  • Arrange for lab and equipment in a timely manner.
  • Demonstrate proficiency and competency in knowledge of Home Care regulatory requirements.
  • Demonstrate competency to follow up on requests and concerns expressed by referral sources in a timely manner.
  • Demonstrate competency to give detailed reports to appropriate personnel considering patient needs through collaboration with the Home Care Team.
  • Visit referral sources as per established standards.
  • Demonstrate proficiency and competency in knowledge of Home Care regulatory requirements.
  • Demonstrate competency to complete timely and accurate activity reports.
  • Demonstrate competency in required computer skills.
  • Demonstrate competency in the ability to analyze, interpret, and report data.
Qualifications/Requirements:
  • One (1) year experience in a Certified Home Health Agency or Discharge Planning/Case Management role.
Education:
  • Graduation from an accredited school of Nursing required.
Licenses/Certifications:
  • Current registration/license to practice as a Registered Professional Nurse in New York State and BLS required.
