Home Care Nurse

ArchCare is seeking a skilled and compassionate Home Care Nurse to join our team. As a Home Care Nurse, you will be responsible for assessing the physical and functional needs of our participants, developing and implementing individualized care plans, and coordinating with other healthcare professionals to ensure the highest level of care.
Key Responsibilities:
- Conduct thorough assessments of participants' physical and functional needs, including psychosocial status and home safety.
- Develop and implement individualized care plans that address participants' needs and goals.
- Coordinate with other healthcare professionals, including primary care providers and center nurses, to ensure seamless care delivery.
- Direct the provision of personal care, nursing rehabilitation tasks, and other work of Home Health Aides and Geriatric Caregivers in the home.
- Document changes in participants' conditions and details of care provided, ensuring compliance with Agency standards and policies.
Requirements:
- Current NY State Registered Nurse License
- Valid Driver's License and Transportation for home visits
