Job SummaryWe are seeking a highly skilled and compassionate Registered Nurse to join our team as a Hospice Case Manager. As a key member of our interdisciplinary team, you will be responsible for planning, coordinating, and providing high-quality hospice care to our patients and their families.
Key Responsibilities- Coordinate hospice care services with other team members and patients/family members to ensure needs are met.
- Evaluate the effectiveness of hospice services and make recommendations for improvement.
- Perform evaluations and informational visits as needed to assess patient needs and develop individualized care plans.
- Utilize the nursing process to provide professional nursing care and consult with the attending physician and Hospice Medical Director as needed.
- Coordinate patient services with other members of the IDG and contracted services as needed.
- Submit complete, accurate, and relevant clinical notes in a timely manner and participate in IDG and case conferences.
- Take responsibility for ensuring patients' plans of care are kept current and perform on-call duties as needed.
- Actively participate in QAPI meetings and activities and continually strive to improve nursing care skills through education and professional development.
- Graduate from an accredited School of Nursing.
- Currently licensed to practice nursing under the provisions of Article 1 of Chapter 26 of Title 43 of the Official Code of Georgia.
- One year of RN experience, with hospice experience preferred.
- CPR certified to care for patients without DNRs.
- Excellent assessment skills and problem-solving skills.
- Ability to communicate well, both verbally and in writing.
- Must have no exceptions to RN license and be able to administer pharmaceuticals, perform medication reconciliation, and any nursing procedures or special treatments needed within the scope of nursing practice for the state.
