Social Services Coordinator

We are seeking a highly skilled Social Services Coordinator to join our team at GeorgiaGov. As a key member of our multi-disciplinary team, you will provide a broad range of paraprofessional social services to patients/clients/consumers and their families.
Key Responsibilities:
- Conduct patient/client/consumer intakes, participate in treatment team planning, and perform crisis intervention
- Assist in dealing with personal and social problems
- Provide supportive counseling to consumers and families and/or serve as a liaison for social services and may perform case management duties
- Assist clients in locating and utilizing community resources including legal, medical, financial assistance, and other referral services
- Complete patient/client/consumer intakes based on interviews with patients/clients/consumers, their families, significant others and appropriate community agencies
- Counsel clients living in group homes and halfway houses; assist in pre-release and release planning
- Implement and organize the delivery of specific social services within the community
- Implement life skills workshops and programs in behavior management, youth services, community and social services
- Maintain contact with other social service agencies and health care providers involved with clients to provide information and obtain feedback on client's overall progress
- Monitor patient's/client's/consumer's progress towards treatment goals
- Maintain program statistics for purposes of evaluation and research
- Prepare intake reports and case history records and assist with discharge planning
- Provide short-term case management and referral services to clients with emergency situations
- Actively participate in assigned and appropriate meetings as required to discuss needs, goals, objectives, and outcomes of services, quality assurance, etc.
- Provide education to client and family
- Flexible and adaptable. Be willing to work at multiple site
Requirements:
- High school diploma or GED
- Associates degree in a social services related field from an accredited college or university
- Two (2) years of experience in a Social Services Tech 1 position
- Two (2) years' experience in Behavioral/Mental Health Field
- At least 21 years of age
- Possess a valid driver's license
- Outstanding interpersonal skills and the ability to work with patients of all ages
- Submit to and pass a pre-employment drug screen and background checks
