Nurse Practitioner

We are seeking a skilled Registered Nurse to join our Acute Rehabilitation team at Elmbrook Memorial Hospital in Brookfield, WI. As a key member of our healthcare team, you will provide direct nursing care to patients in accordance with established policies, procedures, and protocols.
Responsibilities- Implement and monitor patient care plans to ensure optimal outcomes.
- Monitor, record, and communicate patient condition to ensure seamless care coordination.
- Collaborate with interdisciplinary teams to provide comprehensive patient care.
- Assess and coordinate patient discharge planning needs to ensure smooth transitions.
- BLS Provider certification required within 1 month of hire date.
- Registered Nurse licensure from the Wisconsin Board of Nursing or current home state license required.
