Job Overview

The Clinical Social Services Specialist is responsible for conducting thorough evaluations and formulating a care strategy to meet the psychosocial requirements of patients and their families. This role also involves the implementation, coordination, and assessment of program services while guiding families on available support resources.


Key Responsibilities
  • Conducts detailed evaluations and formulates a comprehensive care strategy addressing the psychosocial needs of patients and their families.
  • Delivers direct patient support within the appropriate licensed field.
  • Assists patients and families in accessing community resources, including support for insurance applications, transportation, and lodging.
  • Engages in professional development opportunities related to social work and rehabilitation services.
  • Provides information regarding external services for patients who do not qualify for in-house assistance.
  • Acts as a liaison for Medicaid's Medical Transportation Program to confirm eligibility for patients.
  • Effectively communicates the social, psychological, and emotional needs of patients and families to the assigned care team.
  • Consults with the team immediately upon identifying any concerning changes in patient or family status.
  • Participates in the external needs committee to secure medically necessary equipment for underinsured or uninsured patients.
  • Maintains documentation of social work-related follow-ups, communications, and meetings within the electronic medical record (EMR) system.
  • Handles administrative tasks such as sending and receiving faxes and scanning documents into the EMR.
  • Conducts home visits as requested by the medical and therapy teams.
  • Attends meetings as needed based on family and social work requirements.
  • Ensures confidentiality of patient information in compliance with HIPAA regulations.
  • Coordinates informational sessions or services for families in collaboration with interdisciplinary teams and external organizations.
  • Identifies departmental needs and contributes to the development of new programs.
  • Performs additional duties and special projects as assigned.
