Senior Outreach Coordinator for Homeless Services

We are seeking a highly skilled and compassionate Senior Outreach Coordinator to join our team at Wellness Equity Alliance LLC. This is an excellent opportunity for individuals who are passionate about addressing health disparities and improving the lives of vulnerable populations.
Job DescriptionThe ideal candidate will possess strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders, including unhoused individuals, healthcare providers, and social service agencies. As a key member of our Street Medicine team, you will be responsible for conducting outreach and engagement activities, assessing patients' needs, and developing individualized care plans to address their medical, behavioral, and social determinants of health.
You will also provide advocacy services to help patients secure shelter or permanent housing, collaborate with local housing/shelter providers, and participate in program/service audits to enhance the effectiveness of our service model. Your dedication to serving the most vulnerable members of our community will make a tangible difference in their lives.
A competitive salary of $65,000 - $75,000 per year, commensurate with experience, will be offered to the successful candidate. Requirements- Bachelor's degree in a related field (healthcare, social work, etc.) or equivalent experience;
- At least 3 years of experience working with vulnerable populations, preferably in a homeless outreach or street medicine setting;
- Strong knowledge of public health/social program services for the unhoused;
- Demonstrated ability to provide guidance and positive customer service using a patient-centered approach;
- Skilled in computer applications and electronic medical records (EMR);
- Must possess a valid driver's license;
Prior experience as a Certified Health Worker (CHW) or Certified HIV/STD Counselor is highly valued, as is proficiency in Spanish or other languages spoken by LatinX communities.
