Care Coordinator for Homeless Services

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Good Seed Community Development Corporation. In this role, you will provide high-quality case management services to homeless individuals and families in Los Angeles County.
About the Role:
- You will work under the direction of our Program Manager to develop and implement individualized service plans for 25-30 program participants.
- Collaborate with participants to maintain housing stability goals and ensure they receive necessary support services.
- Develop professional relationships with participants and manage a caseload of 25-30 individuals.
- Provide outreach, housing navigation, and housing location services to support participant needs.
- Link participants to health, mental health, and substance use services as needed.
- Assist participants with employment, benefits, and educational opportunities.
- Accompany participants to appointments with care providers and help them maintain treatment regimens.
- Facilitate access to mainstream benefits, including Medical, GR, SSI, and other essential services.
Requirements and Qualifications:
- Dedication to our mission, vision, and values.
- Knowledge of surrounding poverty, homelessness, and social factors related to each dynamic.
- Ability to understand participant needs and develop collaborative goals towards greater self-sufficiency and independence.
- Familiarity with services for homeless individuals and families throughout Los Angeles County.
- Understanding of challenges associated with homelessness, including chemical dependency, mental disorders, physical health concerns, violence, and isolation.
