Homeless Outreach Nurse

We are seeking a compassionate and experienced Nurse Outreach to join our team at Special Service for Groups, Inc. - HOPICS. As a Homeless Outreach Nurse, you will play a vital role in providing high-quality patient care to individuals experiencing homelessness, while working collaboratively with our multidisciplinary team.
About the Role
This is a field-based position that requires a strong commitment to working with vulnerable populations. You will be responsible for conducting home visits, assessing clients' medical needs, and developing individualized treatment plans to address their unique health concerns. Your expertise will also be essential in promoting health education, disease prevention, and harm reduction strategies among our client population.
The ideal candidate will possess excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment. You will be expected to maintain accurate records, collaborate with community partners, and participate in ongoing professional development to stay current on best practices in homeless healthcare.
Responsibilities
- Conduct comprehensive health assessments, identify, and prioritize medical needs, and develop individualized treatment plans.
- Provide hands-on care, administer medications, and educate clients on healthy behaviors and lifestyle choices.
- Collaborate with community agencies, hospitals, and clinics to ensure seamless transitions of care.
- Maintain accurate and timely documentation, adhering to HIPAA standards and agency guidelines.
- Participate in multidisciplinary team meetings, sharing knowledge and expertise to inform clinical decisions.
- Stay up-to-date on local resources, services, and trends affecting the homeless population, ensuring our team's responsiveness to emerging needs.
Requirements
To be successful in this role, you must hold a current RN license, have experience working with homeless populations, and demonstrate a strong commitment to cultural competency and social justice. A valid California driver's license, CPR certification, and TB test clearance are required. Familiarity with electronic health records (EHRs) and Microsoft Office is preferred.
