About the Opportunity

We are seeking a skilled LPN to join our regional treatment team in Summit County. As an integral part of the team, you will work closely with behavioral health providers, primary care providers, and other community agencies to coordinate multiple health care services and provide medication management, ongoing evaluation of client progress, and education to the treatment team.

Key Responsibilities
  • Assist the RN Care Coordinator in identifying individuals with risk factors and developing strategies to mitigate them.
  • Act as a liaison between public and private health facilities and service agencies to facilitate referrals and coordination of care.
  • Participate in an interdisciplinary treatment team and utilize resources from other community agencies to achieve treatment goals.
  • Serve as a liaison consultant to the behavioral health treatment team on medication and healthcare issues.
  • Make home visits to administer medication and assess individual health conditions.
  • Accompany clients to medical appointments and provide supportive interaction and education to foster self-management skills.
  • Maintain accurate client clinical records and assist the Director of Nursing in maintaining clinical staffing.

Requirements
  • Graduation from an approved school of Practical Nursing and licensure as a Practical Nurse.
  • A minimum of one year's experience working with individuals with severe or persistent mental illness.
  • Current knowledge of psychotropic medications and recent clinical experience.
  • A car and valid Ohio driver's license, as well as insurability through our agency's insurance carrier.
  • A deeply held belief in our recovery-centered mission and ability to develop positive relationships with individuals with mental illness.
