Job Description

Medi Home Health Agency, a division of Medical Services of America, is seeking an experienced Patient Care Coordinator to join their team in Butler, PA. This role requires an LPN license and involves coordinating patient care activities, ensuring they are appropriate, timely, and cost-effective.

The ideal candidate will serve as a liaison between the Agency and the medical community, establishing new accounts and maintaining existing ones within assigned territories. They will also complete and maintain weekly account profiles, account visit history, and account referral history.

Key responsibilities include:

  • Establishing new accounts and maintaining existing ones
  • Completing and maintaining weekly account profiles, account visit history, and account referral history
  • Maintaining weekly contact and visits with accounts on the account list
  • Acting as a liaison between field staff and physicians in obtaining plans of care and modified orders
  • Receiving information from field staff regarding patient care issues and prioritizing and reporting them to the appropriate parties

Qualifications and Skills

  • LPN license and ability to work in PA
  • High school diploma or GED required, Bachelor's degree preferred
  • Minimum of one year home care experience preferred
  • Experience promoting community awareness as it relates to home health and/or hospice preferred
  • Reliable transportation and company required auto liability insurance
