Key Responsibilities- Provide clinical mental health consultation services to patients referred by the care team.
- Participate in the management of psychosocial aspects of chronic and acute diseases.
- Apply behavioral principles to address lifestyle and health risk issues.
- Consult and co-manage in the treatment of mental disorders and psychosocial issues.
- Advocate for patients who are perceived as difficult to work with or "non-compliant".
- Connect patients to a variety of resources in the community related to social determinants of health.
- Master's degree in a mental health field.
- 2 years of experience in the use of evidence-based counseling techniques, screening, diagnosis, and treatment of common mental health and substance abuse disorders.
- Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC).
- CPR certification from the American Heart Association.
- Act 34, Act 33, and Act 73 clearances.
