Key Responsibilities

  • Provide emotional support and counseling to patients and their families, helping them cope with the emotional and psychological impact of terminal illness.
  • Connect patients and families with community resources and services, ensuring they receive the support they need to maintain their quality of life.
  • Collaborate with healthcare professionals to ensure comprehensive care, addressing the physical, emotional, and spiritual needs of patients.
Requirements
  • Master's degree in Social Work (MSW) from an accredited institution.
  • Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Illinois.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Strong assessment and problem-solving skills, with the ability to think critically and make sound judgments.
