Job Summary

We are seeking a skilled Nurse Practitioner to join our HouseCalls team in Wyoming. As a key member of our team, you will conduct in-home assessments, perform evidence-based practice screenings, and communicate findings to inform member care. This is a unique opportunity to make a meaningful impact on patients' lives while enjoying a flexible scheduling arrangement.

Key Responsibilities
  • Conduct comprehensive in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing as appropriate.
  • Identify diagnoses to inform care management and active medical management.
  • Communicate findings to inform members' primary care physicians of potential gaps in care.
  • Intervene in urgent and emergent situations as needed.
  • Educate members on disease processes, medication, and compliance.
Requirements
  • Active NP license or ability to obtain by start date.
  • Unencumbered, free of any open/unresolved disciplinary actions.
  • ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or ability to obtain by start date.
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions.
  • Access to reliable transportation for travel up to 85% within licensed states.
Preferred Qualifications
  • 1+ years of clinical experience in the highest level of education, clinical setting.
  • Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology.
  • Home Health care or home visit experience.
