About the Job:
This is an exciting opportunity to join our dynamic team as a Healthcare Navigator As a key member of our community services team, you will play a vital role in providing holistic support services to our clients, helping them navigate the healthcare system and access essential resources. Your primary responsibilities will include:
- Conducting comprehensive assessments to identify client needs and develop personalized care plans
- Collaborating with healthcare providers, social services agencies, and other stakeholders to ensure seamless coordination of care
- Providing education and advocacy to clients and their families on healthcare options and benefits
- Connecting clients with community resources and services to meet their physical, emotional, and social needs
Requirements:
To be successful in this role, you will need to possess the following skills and qualifications:
- Bachelor's degree in a related field, such as social work, public health, or healthcare administration
- Minimum 1 year of experience working in healthcare or social services
- Excellent communication and interpersonal skills, with ability to work effectively with diverse populations
- Strong organizational and time management skills, with ability to prioritize tasks and manage multiple projects simultaneously
- Ability to maintain confidentiality and handle sensitive information with discretion
