Job Summary

Centinela Hospital Medical Center is seeking a skilled Discharge Planner I to join its award-winning team. As a key member of the Case Management Department, this role will be responsible for coordinating the various activities of the department under the direction of the assigned Case Manager/Social Worker.

The ideal candidate will have knowledge of Discharge Planning/Utilization Management/Case Management terminology and functions, with experience in basic to intermediate computer skills/knowledge. A High School Diploma is required, and an Associate's degree or LVN license is preferred.

Responsibilities will include maintaining and coordinating Medicaid state submission forms, assisting with case management referrals and authorizations, and developing and implementing discharge plans. The successful candidate will be a team player with excellent communication and organizational skills.
