Position Summary
In this role, the Nurse Practitioner in Primary Care will deliver comprehensive health services to patients across all age groups. Working closely with the medical team, this position adheres to established healthcare standards and practices while providing quality care.
Key Responsibilities
The following responsibilities outline the essential functions of this role. This list is not exhaustive and may be subject to change:
- Conducts thorough medical assessments, including evaluations, diagnoses, treatments, follow-ups, and health education tailored to the patient's age and condition.
- Gathers detailed patient histories and ensures accurate documentation of care provided.
- Orders and interprets necessary diagnostic tests to inform treatment decisions.
- Develops and documents individualized treatment plans, including medication prescriptions based on comprehensive assessments.
- Communicates treatment plans effectively with the healthcare team, including physicians, nursing staff, and case managers.
- Educates patients and their families about care plans, disease management, and medication instructions.
- Provides medical care for patients with acute and chronic conditions in line with accepted clinical standards.
- Delivers health education to patients and families as needed.
- Maintains accurate clinical records, documenting findings and care delivered, including lab and imaging results.
- Teaches patients about health maintenance and provides educational resources as necessary.
- Collaborates with healthcare providers to address patient health needs and evaluate care effectiveness.
- Initiates communication with other healthcare professionals as required.
- Ensures the confidentiality and security of all patient information, using it solely for job-related functions.
- Master's degree in Nursing with completion of an accredited Nurse Practitioner program is required.
- Current state licensure as a Nurse Practitioner is mandatory.
- Active state licensure as a Registered Nurse is required.
- Current Basic Life Support (BLS) certification is required prior to patient care.
- Previous experience is preferred but not mandatory.
- Eligibility for DEA and CDS is required.
- Demonstrates the ability to provide age-appropriate care and understands the principles of growth and development across the lifespan.
- Possesses strong verbal and written communication skills for effective interaction with patients, families, and healthcare teams.
- Maintains current knowledge of hospital safety protocols and mandatory training requirements.
- Ensures immunizations are up to date as per practice guidelines.
Ensures patient safety through active participation in safety initiatives within the hospital and department:
- Identifies and addresses potential risks to patient safety.
- Reports any adverse events or near misses to the appropriate management.
- Recognizes and communicates possible risks in processes and procedures to relevant authorities.
