Nurse Practitioner

We are seeking a dedicated and compassionate Registered Nurse to join our team at Valor Healthcare, Inc. in a Community Based Outpatient Clinic (CBOC) setting. As a valued member of our healthcare team, you will play a critical role in providing high-quality patient care and contributing to the success of our organization.
Key Responsibilities- Assist practitioners with patient care, physical examinations, and ancillary tests.
- Practice specialized skills in patient assessment and develop patient care plans in collaboration with the PACT Team.
- Triage unscheduled patients requesting care and act as a point person within the PACT Team.
- Provide patient and family health education, focusing on self-management, prevention, and wellness.
- Monitor VA discharge lists and contact patients for follow-up appointments.
- Maintain confidentiality of veterans' data and information, adhering to all VA and company training requirements.
- Consult with non-VA providers and facilities as permitted by VAMC.
- Fulfill compliance requirements of the Office of Inspector General (OIG), Joint Commission (JC), Environment of Care (EOC) oversight, lab compliance, and other related items.
- Ensure excellent clinical outcomes through adherence to VA guidelines.
- Provide exceptional customer service to veterans, colleagues, and clinic visitors.
- Participate in clinic outreach events to support enrollment initiatives.
- Support patient enrollment and retention in appointments, follow-up visits, or nurse visits.
- Bachelor of Science in Nursing (BSN) required; Associate Degree RN may be acceptable if VA contract allows.
- Minimum 2-3 years' experience as a registered nurse in a related primary care or ambulatory care setting; government healthcare environment preferred.
- Credentialed and in good standing through the Veterans Health Administration (VA).
- Valid, unrestricted license in the state of desired employment or any U.S. state or territory, depending on VA contractual requirements.
- Current certification in Basic Life Support (BLS) and ACLS as specified by individual VAMC contract.
- Proficiency in written and spoken English.
- Strong computer skills, including EMR experience.
- Energetic and optimistic demeanor.
- Strong service mentality and focus on achieving defined service standards.
