Job Responsibilities
  • Connect Community Members with Resources: Serve as a key liaison between HHN and the community, ensuring access to essential healthcare services and resources.
  • Conduct Outreach Activities: Engage in proactive outreach initiatives to raise awareness about health education and community resources within the local community.
  • Navigate Healthcare Systems: Assist clients in navigating the complex healthcare system, guaranteeing they comprehend their care plans and necessary steps for recovery and well-being.
  • Emotional Support and Resource Facilitation: Provide vital emotional support and facilitate connections to local community resources, including educational, financial, and social assistance programs.
  • Caseload Management and Documentation: Effectively manage a caseload of 60-75 members, accurately documenting dates, time, and nature of services provided to integrate this information into each member's medical record.

Skills and Qualifications

  • Lived Experience: Must possess lived experience that aligns with and provides a connection to the members served, potentially related to incarceration, military service, pregnancy and birth, disability, foster system placement, homelessness, mental health conditions, substance use, or being a survivor of domestic violence or exploitation.
  • Cultural Competence and Passion for Service: Demonstrated passion for community service and healthcare, particularly focusing on vulnerable populations, coupled with cultural competence and shared background with the community served.
  • Exceptional Communication Skills: Possess exceptional communication and interpersonal skills, capable of engaging effectively with diverse groups.
  • Strong Organizational Abilities: Display strong organizational abilities, with the capacity to work independently and as part of a multidisciplinary team.
  • Bilingual Skills and Mobility: Bilingual abilities are highly valued and must be willing and able to travel within Riverside and San Bernardino counties for home visits and community engagement activities.

Compensation and Benefits

  • Estimated Salary: $65,000 - $85,000 per year, depending on qualifications and experience.
