How You Will Make a Difference

As a Care Transition Navigator, you will orchestrate the perfect balance of patient care and operational efficiency, ensuring the patient experiences a smooth transition home. You will be the face of VitalCaring Group when interacting with referral sources, cultivating and nurturing relationships to deliver exemplary care to patients along the care continuum.

To succeed in this role, you will need to be passionate about delivering high-quality patient care, committed to delivering outstanding customer service in every interaction, and solution-driven with a focus on execution and urgency. You will also need to be enthusiastic about being accountable for delivering measurable results within agreed timelines.

Requirements

To be successful in this role, you will need to have a RN license, a valid state driver's license, and reliable transportation. You will also need two years of clinical experience, with one year of home health or healthcare sales experience preferred. Knowledge of business and fiscal management, governmental regulations, and accreditation standards is also required.
