LPN Opportunity with Amedisys
Amedisys, a leading home health and hospice company, is seeking a skilled LPN to join our team. As a Licensed Practical Nurse, you will provide high-quality care to patients in their homes, telephonically, or via video.
Key Responsibilities:
- Perform visits for an assigned caseload of patients and families in their homes, telephonically, or via video.
- Report changes in patient condition/symptoms and coordinate care with oversight from the RN/advanced practice RN.
- Engage patients and caregivers in discussions about their goals of care, advance directives, and collaboratively participate in the development of treatment plans based on the patient's goals.
- Participate in assisting with and giving direct patient care in conjunction with the rest of the clinical care team to ensure interventions are aligned with patients' goals.
- Advocate for the patient and caregivers in liaison with other care providers to ensure care is both coordinated and consistent with the patient's goals.
Requirements:
- Graduate of an accredited licensed practical nursing program or accredited school of vocational nursing.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in a home health, palliative, or hospice setting.
