BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in Newport, Vermont.
Job Summary:We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our clients in their homes. As a Physical Therapist with BAYADA, you will have the opportunity to work with a diverse range of clients and make a meaningful difference in their lives.
Responsibilities:- Conduct home visits to assess and treat clients with physical disabilities or injuries.
- Develop and implement individualized care plans to help clients achieve their goals and improve their quality of life.
- Collaborate with other healthcare professionals to ensure comprehensive care and coordination.
- Provide education and training to clients and their families on proper techniques and strategies for managing their conditions.
- Document client progress and outcomes to ensure accurate and effective care.
- Current Vermont Physical Therapist license.
- Minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
