Job Summary

BAYADA Home Health Care is seeking an experienced Physical Therapist to join our team in Orleans County, VT and surrounding areas. As a home health Physical Therapist, you will be an integral member of a multi-disciplinary health care team providing rehabilitative care and skilled nursing to clients in their homes.

Responsibilities
  • Make home visits to clients in designated geographic territories to assess and develop individualized care plans.
  • Perform diagnostic tests and measurements, such as mobility and strength assessments, to inform care planning.
  • Develop and implement manual therapeutic exercises, gait training, and other interventions to promote client independence and safety.
  • Educate and instruct clients and their families on rehabilitative care and activities necessary for optimal health and safety.
  • Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet.
Qualifications
  • A current Vermont Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
