- Collaborate with primary care clinicians and other cross-organizational areas to meet patient needs
- Utilize technology, including secure email and electronic medical records, to facilitate communication and care coordination
- Adhere to documentation protocols and best practices for daily work logs and internal communications
- Provide exceptional customer service and contribute to a positive team environment
- Must be a Licensed Practical Nurse (LPN)
- Must be comfortable with phone communication and able to work in a fast-paced environment
- Must be compassionate and empathetic towards patients, with excellent customer service skills
- Ability to multitask, prioritize tasks, and maintain accurate records
- Proficiency in using a computer, including Outlook and Microsoft Office programs
- Knowledge of basic healthcare terms and principles is beneficial
