Job Summary

We are seeking a highly skilled and compassionate Case Manager to join our team at AltaPointe Health. As a Case Manager, you will play a critical role in providing exceptional care and support to individuals with mental health needs.

Key Responsibilities
  • Assess consumer needs and develop individualized service plans to ensure optimal outcomes.
  • Provide linkage to community resources and services to support consumer well-being.
  • Monitor consumer progress and adjust service plans as needed to ensure effective care.
  • Collaborate with treatment teams and outside sources to deliver high-quality services.
  • Assist consumers with transportation needs and provide outreach services as required.
  • Document all interactions and services provided in a timely and accurate manner.
  • Participate in Performance Improvement activities and contribute to the development of policies and procedures.
Qualifications
  • Bachelor's degree in a mental health discipline.
  • Certification of case management training by the Alabama State Department of Mental Health and Mental Retardation.
  • Ability to stay organized and manage a workload with multiple consumers.
  • Knowledge of psychiatric disorders, cultural diversity, behavior management, and crisis intervention principles.
  • Knowledge of psychiatric rehabilitation principles, available community resources, and the social security system.
