POSITION SUMMARY:

The Clinical Nursing Manager is accountable for overseeing patient care within the designated area. This role includes ensuring the effective delivery of services by the assigned staff.

EDUCATION/EXPERIENCE:

Candidates must be a Registered Nurse with a valid nursing license in Connecticut and meet one of the following criteria:

  • A master's degree from an accredited program recognized by the National League for Nursing or the American Public Health Association, along with a minimum of one year of clinical experience in a Home Health Agency.
  • A baccalaureate degree in nursing with at least three years of full-time clinical nursing experience, including a minimum of one year in a home health agency.
  • A diploma or associate degree in nursing, complemented by at least three years of full-time clinical nursing experience within the last five years, with at least one year in a home health agency.

Additionally, candidates should demonstrate certification by the American Nurses Association as a community health nurse or have completed at least six credits in community health nursing or healthcare management from an accredited institution within the last two years.

Certificate/License:

A current license to practice as a registered nurse in Connecticut is required, along with individual malpractice insurance.

Alternative to Minimum Qualifications:

Hospice/Palliative Certification is preferred.

ESSENTIAL DUTIES and RESPONSIBILITIES:

The Clinical Nursing Manager is responsible for the oversight of both professional and paraprofessional nursing services, including those provided through contracts to hospice patient families.

This role involves:

  • Assessing and evaluating community and agency needs and resources.
  • Making recommendations to the Hospice Director regarding staffing requirements.
  • Ensuring the clinical competency of assigned staff.
  • Conducting timely assessments and evaluations of staff performance.
  • Maintaining qualitative standards and adhering to Agency quality assurance goals.
  • Encouraging the professional development of staff.
  • Ensuring staff are informed about criteria for all payer sources as applicable.
  • Participating in weekly Interdisciplinary Team Meetings and keeping staff updated on new trends and issues.
  • Providing ongoing case evaluations to ensure clinical outcomes, documentation, and billing requirements are met.
  • Reviewing progress on clinical outcomes to ensure efficient goal achievement.
  • Conducting audits of patient records to ensure compliance with documentation and regulatory standards.
  • Communicating with insurance companies regarding clinical updates as necessary.
  • Collaborating with Hospice RNs and other disciplines to discuss patient care, goal progress, service coordination, and discharge planning.
  • Ensuring timely completion of documentation requirements.
  • Maintaining quality leadership and management objectives.
  • Facilitating open communication between staff and senior management.
  • Implementing agency policies and procedures with Hospice staff.
  • Being accountable for patient utilization and staff productivity, addressing variances with appropriate staff.
  • Processing patient referrals as needed.
  • Participating in the hiring, interviewing, orientation, and termination processes for assigned staff.
  • Engaging in agency committees and focus groups.
  • Managing additional agency programs as assigned.
