Job Summary

We are seeking a skilled Registered Nurse to join our team as a Clinical Transition Nurse. This role is responsible for ensuring a seamless and efficient transition for patients coming in and out of acute care hospital settings.

Key Responsibilities
  • Obtain a comprehensive nursing assessment history and ensure proper inpatient care unit admission.
  • Take appropriate action when needed, such as communicating special circumstances to physicians and/or care team or reporting indications of abuse or neglect.
  • Complete and facilitate the completion of required documentation, such as admission assessments, risk assessments, medication reconciliation, discharge paperwork, and post-hospital documentation.
  • Teach patients/families/caregivers about a patient's medical condition, patient status, treatment plan, medications and possible side effects, and follow-up measures.
  • Coordinate with House Supervisors/Nursing Administration to prioritize workload and identify greatest need areas to ensure smooth patient transitions.
Requirements
  • Basic Cardiac Life Support must be obtained within 30 days of employment start date.
  • Registered Nurse Texas State or Compact State license required.
  • Registered Nurse Diploma, or Associate Degree, or Bachelors Degree.
  • 1-2yrs Admission Discharge Lounge experience or Medical Surgical/Tele experiences.
