Key Responsibilities:

  • Deliver high-quality nursing care to clients in alignment with established protocols and guidelines.
  • Conduct initial and ongoing health evaluations, addressing integrated healthcare needs and identifying obstacles to care.
  • Utilize sound judgment in assessing situations and applying appropriate medical or emergency interventions.
  • Administer medications accurately; document outcomes and monitor vital signs.
  • Observe patients for any changes in health status following medication adjustments or other significant health events.
  • Support clients in lifestyle modifications to promote health and assist in setting achievable goals.
  • Provide tailored client education based on age and health condition.
  • Implement effective infection control measures.
  • Review prior medical records and histories to inform care scope.
  • Communicate effectively with clients, colleagues, and community partners regarding client needs.
  • Participate in required staff meetings to ensure program compliance.
  • Coordinate medical appointments, referrals, and collaborate with community healthcare providers.
  • Manage laboratory work and communicate any unusual results to clients.
  • Employ de-escalation techniques to handle challenging client interactions safely.
  • Maintain accurate and timely documentation of client care in accordance with organizational policies.
  • Verify client payer sources and secure necessary authorizations as per program guidelines.
  • Perform additional duties as assigned.

Education and Experience:
Must hold a Registered Nurse license in the State of Florida. Experience in primary care, mental health, or substance abuse settings is preferred. Current BLS or CPR certification is required as per program standards. Additional qualifications may be necessary based on specific program requirements.

Knowledge, Skills, and Abilities:

  • Strong understanding of healthcare delivery principles and practices for diverse populations, including mental health and substance abuse.
  • Proficient in computer skills, particularly in Emergency Medical Records.
  • Ability to quickly assess situations and apply sound judgment in medical or emergency scenarios.
  • Capacity to foster a collaborative therapeutic relationship with clients and their families.
  • Excellent verbal and written communication skills.

Special Requirements:

Must possess a valid Florida driver's license, maintain an acceptable driving record, and provide proof of personal automobile insurance if required to operate an SMA vehicle or use a personal vehicle for work purposes. Completion of mandatory background screening prior to employment is required. Must complete SMA training within the first six months of employment and as needed thereafter. Current BLS or CPR certification is required as per program standards.

Physical Requirements:
Must have the mobility to bend and reach throughout an 8-12 hour workday. Ability to lift a minimum of 10 pounds. Visual and auditory acuity must be sufficient to assess, intervene, treat, and document client healthcare needs. Fine motor skills are necessary for accurate charting and correspondence. Must have the endurance to work 8-12 hour shifts, with variability in hours and days off. Additional hours may be required, and work locations may vary.
