Compassionate End-of-Life Care Manager

We are seeking a compassionate End-of-Life Care Manager to join our team at SouthEast Hospice. As a key member of our care delivery team, you will be responsible for providing exceptional care to patients and their families facing terminal illnesses.
Job RequirementsThis full-time position is an excellent opportunity for a dedicated nurse to make a meaningful difference in the lives of our patients and families.
- Main Responsibilities:
- Develop and implement comprehensive patient care plans to address identified needs and improve outcomes.
- Deliver direct care to patients as prescribed in the interdisciplinary plan of care to maintain comfort and quality of life.
- Prioritize patient care by coordinating community resources and other agency disciplines participating in care delivery.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions.
- Actively participate in interdisciplinary group meetings and contribute to care planning and coordination efforts.
- Maintain accurate records and provide regular updates to patients and families.

Compensation and Benefits:
- Salary Range: $65,000 - $85,000 per year based on experience.
