ROLE PURPOSE:

Deliver nursing care and services in accordance with the standards and scope of nursing practice as defined by state regulations, policies, and protocols. Maintain a positive, empathetic, and professional demeanor towards all patients and prioritize their safety.


Key Responsibilities:

ESSENTIAL DUTIES:
  • Monitor patient health and provide basic care in alignment with the nursing process within the LPN/LVN scope of practice, achieving care plan objectives.
  • Exemplify customer service expectations and demonstrate urgency regarding patient safety.
  • Collaborate with team members to address patients' physical, biopsychosocial, age, developmental, and cultural needs, resolving any complaints effectively.
  • Ensure patient comfort by implementing individualized nursing interventions as outlined in the care plan.
  • Apply suitable interventions to support patient wellness and maintain health.
  • Engage with patients regarding their care, addressing concerns and reporting any changes to registered nurses and physicians.
  • Communicate effectively and professionally with patients, visitors, colleagues, and other treatment team members.
  • Administer medications safely, adhering to policies, procedures, and physician orders.
  • Uphold professional nursing standards in accordance with facility policies, external governing bodies, and performance improvement standards.
  • Maintain thorough and accurate documentation of patient symptoms, responses, and progress.
ADDITIONAL DUTIES:
  • Perform other tasks and responsibilities as assigned.
Qualifications:

EDUCATION/EXPERIENCE/SKILLS:
  • Graduation from an accredited nursing program is required.
  • Preferred: One or more years of nursing experience in a clinical setting relevant to the services provided by the facility.
CERTIFICATIONS/LICENSES:
  • Current LPN/LVN license as mandated by state regulations.
  • CPR and de-escalation/restraint certification required (training provided upon hire).
  • First aid certification may be required based on state or facility guidelines.
